Required Documents

Depending on the information submitted during registration, parent(s)/guardian(s) may be required to submit additional health forms that require physician signature. Please check the "Required Forms and Documentation" or navigate to the "Required Documents" section of TADS.

Required Forms can be downloaded and printed. After completion, users can sign back into their mytads.com account to upload the signed forms or signed forms can be dropped off at the school front office.

Required forms are due by the end of August.
